Dorsolateral Prefrontal Cortex

DLPFC — planning & initiation

Key for working memory, organisation, and goal-directed behaviour. People may understand what to do but fail to initiate or sustain it. The classic “knows but doesn’t do” presentation.

Orbitofrontal Cortex

OFC — judgement & inhibition

Regulates social judgement, inhibition, and emotional decision-making. Damage leads to impulsivity and poor real-world judgement — yet often appears fine in structured assessments.

Ventromedial Prefrontal Cortex

vmPFC — motivation & insight

Controls motivation, emotional weighting, and self-monitoring. Linked to apathy and reduced drive. Patients may verbally endorse goals but not act on them.

Structure over persuasion

Routines, visual prompts, and environmental cues work. Verbal reminders and arguments often don’t — they place the executive burden back on the person who cannot carry it.

Observe, don’t just ask

“Can they carry this out?” matters more than “can they explain it?” Real-world performance in unstructured settings is the evidence — not the conversation in your office.

Expect variability

Fatigue, time of day, stress and novelty all affect performance. A good day does not mean the difficulty has resolved. A bad day does not mean the person isn’t trying.

Transitions are high risk

Discharge, step-down and service withdrawal are the moments the FLP becomes most dangerous. Performance in a structured setting does not predict performance after leaving it.
